Appointments

Our office operates on the appointment system. Please be on time for your appointment. Family Medicine Associates requests that you call the office 24 hours prior to your appointment if cancellation or rescheduling is necessary.

New Patients
Please complete the registration and release forms available for download from our forms page prior to your appointment. Please arrive 15 minutes prior to your appointment in order to complete the necessary documents before seeing the physician. Also, please bring current medications, insurance cards, photo ID and co-pays.

Secure Patient Portal
Family Medicine Associates has a SECURE Patient Portal for our patients to access important medical information, send messages to the office, request appointments, and refill prescriptions. Our hope is that you use this portal to streamline your healthcare. We look forward assisting you.
